My family member is placed at Brookside on the Azalea unit. I was nervous with reading the reviews on here to have him placed at Brookside but the staff on the Azalea unit have been phenomenal. Matt, Marie(CNA), Jamie, and Ken (Nurse) all have been so helpful and I appreciate them all helping me keep up to date on how my brother is doing. I do not live close enough to visit weekly so it’s a great help. Matt responds to all my emails at all hours of the night and does face time calls with my brother and I two times a week so I can see his face. It is so comforting knowing he’s placed at Brookside and has such a great bunch of individuals that work with him.
The pictures tell my story. This is the quality of care and conditions in their Long Term Medicaid Unit they call the “Diamond Ward” . It’s my dads roomThat he was just placed in after leaving their bright lit rehab private pay unit. My first visit with my mother who was in tears I’ve filed a formal complaint and I recommend anyone reading this do the same. This is a complete horror show. Dead stop. PERIOD.
I cant give any lower than one star, Its not a Available. I assume these reviews are referencing the front part of the facility the private pay part which is bright and shiny and clean with clean sheets smells nice and is fairly well staffed as opposed to the part that contains the patients that are not fortunate enough to have private pay . The other half of the facility looks smells and is dirty as any inner city homeless shelter and the level of care is way below acceptable
I had a loved one placed here. They were on the Azalea unit and the care was excellent . I must of called the Nursing Team( Marie, Kenley, Lorrie) & Matt over 100 times a day. They always answered all my questions and were very helpful to me during this difficult time. They even had pizza parties for my dad every Friday which made his whole day.
This is a very good facility. If you are willing to put some time and effort into communicating the needs of your loved one, you will get excellent results. Not all areas of the facility are updated, but the care is decent in all areas of the facility. The skilled nursing staff conduct themselves in a very professional manner. Rehab and physical therapy are also excellent. All members of the team in admissions are excellent communicators and try very hard to understand the needs of prospective clients. They also provide good followup to insure a smooth transition.
Unexpectedly my father-in-law needed to be placed in a long term nursing home. After much research we looked into Brookside and so happy we did. The section that my father-in-law was place is NOT the updated portion of the facility, but is nice enough and we really couldnt ask for better care. The admissions department made his admissions very easy and took the initiative to take care of everything. The nurses on staff are very friendly, caring, patient, attentive and knowledgeable. We are so happy we chose this facility and he has settled in very nicely. I would highly recommend looking into Brookside if you are considering a nursing home for a loved one.
I would not recommend this place to anyone. It was an absolute nightmare for the 100 days my grandfather was there. No communication between staff, they did not help him when he called for assistance. He received pneumonia 2 times and a uti under there care. When you question anything you get an attitude from administration with lots if promises but no follow thru. Staff on multiple occasions was verbally abusive towards him (mostly overnight staff). We filed a formal complaint but nothing will come if it. The place looks nice and clean but the looks are deceiving. I can count on one hand how many aides were decent. If you or your family member have to go here then my suggestion is to be on top of them everyday. My grandfather left worse off then when he got there. He was able to walk with a walker pretty well, now he can take maybe 3 steps and is in a wheelchair/chair most of the day. I blame them, they left him in his wheelchair all day and only worked with him when he had PT. Horrible place, stay away!
This Nursing home is Not 4 or 5 Star nursing home. I review this nursing home for my mother and I am very disapointed in their care. They have a lot of problems, the nurses are very disrespectful. They leave patients on the bed pans for hours, and on top of everything she has fell out the wheel , coming from theraphy unattended. They never answer the bells beside there beds. This is a disaster. PLEASE DONT NOT LOOK AT THE REVEIWS LIKE I DID. ON TOP OF EVERYTHING SHE WAIT 2 WEEKS FOR A SHOWER!!
Brookside Multicare has been a Godsend. I fell down at home due to bulging disks in my spinal cord and ended up in the emergency room for almost 3 days. It became abundantly clear that i was weak and I needed more comprehensive rehab that the hospital could just not provide. After researching different facilities online, my family and I decided on Brookside in Smithtown, NY.They were very cordial while admitting me, Mike especially made me feel welcome and I felt like they genuinely wanted to comfort me.The first night was a little rough for me but with help from Fran (concierge) who checked in on me almost daily, (to make sure I needed nothing) my stay became easier and more pleasant.With the care of Lisa n Kelly(head nurses) as well as Diane n Reggie (nurses) I was able to calm down and feel safe. Christina (nurse aide)was also very sweet to me.The next morning I met Mr. Ron Tingling (nursing aide) who faithfully woke me up and offered to help me get ready for rehab every morning. This person was the most kindest/caring person who was instrumental in helping me survive this ordeal with dignity and grace. He has allowed me to endure my three week stay. Thank you Ron.Gabby the social worker was very informed and genuinely cared about helping me navigate through the heath insurance maze.Physical therapy and Occupational therapy were both fun and arduous. Lauren Bivona and Ragi were in charge of getting me back in shape. They were both sweet and tough while motivating me to MOVE AGAIN. They helped me regain my strength & balance. In fact all the therapists showed care and professionalism not just to me but the other residents/patients as well. David & Rokam performed the initial assessment of my situation and set the tone for my road to recovery. Louis, Kay and Jennifer deserve a shout out for all their help in the gym. Btw, the view from the gym while utilizing all the state of the art equipment, is both peaceful and inspiring as it showcases a beautiful multi-tiered waterfall, landscaping and a putting green!The only negative thought I’d like to share is that sometimes it gets very noisy because of the emergency call bell in each room. If they would change this system to a visual one with lights instead of the very loud bell it would be much more peaceful and restful.The meals provided were better than expected. The bottom line is that I would recommend this facility to everyone including my family.
Worst experience had to call ambulance at 430 am. My mother who is totally competent was laying in bed in feces no pain medication they said the key didn’t work. She had just 3 days before had a rod put in her leg. Everything was just horrible. Don’t ever go there or send a loved one
When my husband was originally admitted to this nursing home it was filthy and nursing staff was very rarely available to help him ( he was an amputee ) because he needed help going to the bathroom , showering etc.. Basically ignored him for the most part & with the lack of interest caused many falls . He was taken to the hospital numerous times , once he stayed over 1 week which caused him to be discharged from Brookside . After his release from the hospital & returned he did not receive his medication for an entire week end & when I continued to ask the nurse to call the on call doctor , she said she did & he refused to sign an order , causing my husband to go through withdrawal ..Before fixing the place up to look “great”.. it was a mess .. urine not cleaned off the floors , dirty linen , never returning all of his clothing , head supervising nurse talking about him where he heard her , totally overall unprofessional and the lack of safety protocols..I would never recommend putting anyone in this disgusting place .. sure” looks beautiful “ but reality is the patients are not taken care of properly unless they are favorites .. I personally seen for myself :Elderly tied in a wheel chair all day by the nurses station “ to be watched closelyPatients laying on filthy urine wet linenPatients screaming for staff & being ignoredOther patients stealing from patients & nothing getting doneNot getting meds as scheduled unless screaming for the nursing staffAll excuses were “they were under staffed “ & that allowed patients to be totally ignored while I sat & watched as I visited daily.It so happens my husbands last admission to the hospital was his last , he passed last year ..Terrible experience & wouldn’t wish this on any human being 🙏
My mother in law is a permanent resident here and I have to say she is being well taken care of, we love the staff and they love her, she is in the new wing and gets to use the gym also. For the most part she like the food she is being served and her room is clean. We have no complaints thank God
Excellent facility for rehab. Extremely professional and knowledgeable physical therapy staff. Nursing and aide staffing very kind and caring. Administration staff superb to work with. Two suggestions: upgrade cable tv system and modify room alarm system. Extremely loud and annoying. Overall, great experience considering circumstances.
I was in an emotional😩😞😭 turmoil when selecting a rehab for my gran. Jennifer the liaison contact at Brookside ( formerly known as Avalon ) with Stonybrook Hospital was sympathetic and attentive. ❤Upon grans arrival, all was well initially with staff and the level of care. There was one assistant (CNA I believe) that was unkind 😡when I wasnt present. I addressed it IMMEDIATELY☠☠ and there were no further issues.The nurse Diane❤, older slender Spanish Rehab guy Louie❤(think thats his name) and CNA Andrew❤ were PHENOMENAL . Aside from the single rotten apple😒 , the Brookside Rehabilitation Team treated gran well🙌.Cons:Theres a stench that will make you wonder whose diaper needs to be changed. 😩Severely understaffed . Once the bell is rung for assistance , the wait of 30 plus minutes was a bit excessive in my opinion.Food and coffee WAS ALWAYS cold .Upon arrival of her food , it ALWAYS had to be warned up. More times than not , most of the coffee was spilled in her tray .Additionally, a boiled egg she recieved for breakfast was BLACK inside😳.Cold coffee, burnt toast and spoiled egg.YUM *insert sarcasm here*The meals leave much to be desired.Overall, its not a terrible place for loved ones . Especially when surprise visits are done with frequency.
DO NOT SEND YOUR LOVED ONES HERE! The nurses and aids neglect their patients, make fun of them, mock them. My mother begged for a week straight to be sent to the hospital and they said that she did not need to. She became so sick and weak she could not feed herself and two of the aides there said to her we are not going to feed you like a baby so she did not eat for days. I finally threatened the night supervisor to call 911 to have her brought to the hospital. So they sent her there finally. We find out that she has congestive heart failure. If she had stayed one day longer she would have died. THIS PLACE NEEDS TO BE SHUT DOWN!!!
PT is outstanding here! Food is hit or miss, but we are not here for the cuisine...Rooms are 1/2 a size too small, and equipment is outdated. They still use temp-a-dots, which kind of went out in the the 80’The biggest problem is getting a diaper change; I rang at 0715, and at 1000 I am still waiting.
Avalon Gardens Health Cere and Rehabilitation Center is the most patient centered, family oriented facility with skillful clinicians poised to meet the needs of all individuals irrespective of their medical conditions. If you want the best social support, nursing care and active physical rehabilitation then Avalon Gardens Health Care Center is the place to be.
Spoiled milk for breakfast Rotten eggs the menu doesn’t change week to week it’s the same thing weekly they tell you it’s not a jail but it is a jail do not trust a video or brochures there all Lies nurses and aids a great there take care of the wound and Med A+++ with no problem (Administrative staff are bullies)

2022-05-27I left this facility a month ago. Food is very good. Nursing staff was very helpful in my time of need. Got my pain meds on time. I was treated with respect. Rehab really pushed me and helped me to get back to my full potential. Workers here were very friendly and accommodating. Very clean. I sincerely thank you Brookside team

2022-05-26First and most important I would give a five star for everyone’s behavior care and concern for the people that enter the rehab. It was a hike for me to care for my mom and let my mom know that I was there for her. Going up once a day was not good having my five children and six grandkids not see her was not good. It was out-of-the-way from my home, and her family. The food was good the care and friendliness of the staff was very important. Including my mom was very confused loved the staff and care. The food was so so. She wasn’t there long enough for me to see what they did with her on a daily basis. I do feel that a little pushing needs to be done with physical therapy. When the patient says she doesn’t want any never listen just tried the latest thing possible to get them moving . I needed to get her closer to home. Thank you all for your care concern and friendliness. Especially the one who loves Starbucks. I agree I wish it never moved from deer Park Avenue. :)

2021-12-24I’d leave another 1/2 star if I could for the mostly good staff. Too many issues with this place. Bruises on moms arms & wrists. The odor in the hall and bathrooms. It seemed fairly clean but the linens were old. It is way understaffed, mostly at night and it’s a ghost town on the weekends. People call out and there isn’t anyone to answer. This is the owner fault. Covid times or not they should be staffed properly. They are not equipped for dementia, let alone advanced dementia. The food isn’t great so I would make my own food , bring it & feed it to mom. They don’t have the time/staff to administer to extra need patients. I had to be there everyday for several hours to advocate for mom. She was continent when admitted & immediately put on depends instead of a toileting schedule .now she’s incontinent! Left unattended for hours in a wheel chair -at times becoming a fall risk. It took almost 4 days for her clothes to br labeled. They said she had pneumonia but when admitted to the hospital few days later she did NOT have pneumonia. I hesitantly placed my mom here thinking the daily rehab would help but I found her health mentally & physically deteriorated, I found myself there daily & for many hours. I recommend you to be there constantly to advocate for your loved one. In fact, it’s best for them to be at home & not be placed here. Unfortunately, there are worse places than this & it’s a shame that elders especially those with dementia aren’t given the proper attention. I wanted mom out but there was always an issue as to why she couldn’t be released. She’s home now but I regret my decision immensely for her placement there.

2022-05-24A close elderly relative was brought here from a local hospital, and was treated terribly. We actually filed a complaint with Dept of Aging. This place deserves to be shut down.We arranged for a time for him to be discharged, and booked home health aides and nurses for him, but they sabotaged that by telling the nurses that he needed 24-hour care. So we had to scramble and find another nursing agency from their preferred list.The social workers did nothing to help him go home with arranged care, and kept stalling and trying to find excuses to keep him there. We had to arrange the care ourselves and then at the last minute they claimed they needed an additional 3 days to discharge. Its as if they wanted to keep him there to get as much money as they could from his insurance.He had diabetes and they werent managing his blood sugar levels or giving him the correct medication.They had their own psychologist do simply memory tests and declare him as having dementia. They blamed all his complaints of mistreatment on dementia and gaslighted him. They provoked him to be angry and then blamed that on dementia. (When he was released neurologist examined him and said he didnt have dementia and actually scored better than most people his age.)He needed bandages treated 2x/day but often went days without the bandages being changed. They would lie to him and his family about what the hospital discharge papers said, and what they did.They would take his laundry and delay getting it back to him, lying and saying that it was returned to him when it wasnt. Wed have some of the nurses tell us that it wasnt returned, others saying it was and that he was senile.The meals were usually cold, sometimes they seemed to have have been frozen and not even defrosted or ever heated up. Sometimes hed send the food back to be heated up and theyd simply take the food away and hed go without a meal.

2022-02-04My opinions have changed so much about Brookside. My sister is placed on the Azalea unit and for years she has been there. It was always the an amazing team and the staff went above and beyond ( Matt/Lorrie/ Marie) used to call me with regular updates and the communication was excellent. Brookside has lost valuable players to my sisters team who promoted the quality care that we needed. I have not been able to get through to my sister and continue to struggle to hear any updates. Not the same place without these people who made the unit what it was! Looking to move my sister as soon as possible

2022-04-08I agree with a lot of the other people that rated this. I was in the Rehab for maybe 1 week and a half or two weeks after having a Stroke. I was experiencing migraine headaches and couldn’t eat anything most of the time I was there. And the food really didn’t appetizer me at all. So needless to say I was transferred to St. Catherine’s of Sienna when I was having chest pains and very dehydrated. I did have some really good nurses that were very caring that were and got me gingerale to drink because I wasn’t eating anything just sipping on ginger ale from time to time. I was really dehydrated and had Cardiac problems already.

2022-05-12Been there five times. Each successful. The staff is very caring and the charge nurse is just angel.everone is caring down to porters.foodnhas a lot to be desired. Physical therapy best of all rehabs. Many activities if you want.

2022-04-17I wish I could give no stars. Place is seemingly great at first. My mom had to go here because it was the only place that insurance would cover. She was treated horribly. No room phone so no way to contact her but they can relay the message because she cant walk and use the phone herself. She also doesnt get to pick what she wants for dinner, even if she doesnt eat fish then you dont eat. She told me her shower experience was traumatic as hell, she was sitting on her oxygen the whole time and couldnt breathe and they still forced her to wash herself. The whole time upon visit no one checked on her once, her sheets were soiled and she was very neglected. When my sister asked for a new pull up, since theyre too lazy there to wipe her as shes capable to use a bedpan, they tossed the pull up at her knowing my sister would do it for them. Terrible place. Save your family and get them out now!

2022-06-04The first night my wife was moved from emerald which seemed nice, except for all night ringing call bells left unanswered, to a room on Broadway where she was packed in a 2 person room as a third person, her cell phone was stolen by a staff member . She did not misplace it, as the blue tooth in someone’s car kept picking it up. From a girl who worked as an aide there, stealing by the staff is quite common. What is this jail for old sick people? Lowlifes work there.

2021-12-15Don’t let the pretty pictures posted here fool you!!This palce does not communicate well with family, we could NEVER get anyone to actually answer the phone, no matter when we called- I actually had to return their call, and still no one picked up the phone. A horrible facility, I would have given them two stars but the night staff earned them a star. Dad was placed here after having health issues and needing rehab. While there, after two weeks, he was diagnosed with COVID- he was Covid free when leaving hospital. As a result of this Covid infection, 4 other family members came down with Covid. It was not even suggested that he be taken to hospital for antibodies treatment. Family member had to order him to be transferred to hospital to get the antibodies treatment.Today when family member goes to check on patient, they then proceeded to inform him that he now has pneumonia and there is nothing they can do other than make him comfortable. It is ridiculous, the lack of communication between the staff and the family. The day crew is horrible. The only saving piece of this place is the night and weekend staff, they were fantastic. I can’t say enough about them.The facilities cleanliness is lacking as well. I found myself cleaning the patients room. The sheets used were all cast offs from other healthcare facilities and were very threadbare to say the least. It would have been. Ice if when the meals were served that they would have been warm. The only warm meals family member got were the ones we brought in for them.The best advice I can give is to avoid this place.
