I was called by the case manager of this facility letting me know that they had a bed for my mother and they knew the problems I was having with Hudson Park and St Peters hospital so I transferred her out of pocket just for her to be left alone, in soiled clothes that were not hers and sitting in her own mess. I was MORTIFIED. I called to have them have their notary available so I could get some of her affairs in order just in case. 45 minute drive up there just to have them tell me they just gave her medicine that makes her groggy so she was in no condition to sign. A week later I get a call at work telling me that they found her unresponsive and were sending her back to St Peters hospital. The end result was not supposed to happen.😔
Fulton Center is seriously under staff. The food is terrible. Nothing for activities because they take the staff from there to help on the halls with patients. On the weekend the meds are late and if youre in pain forget it, b/c its takes the rest of the day catching up.
I wish this place was closer to my home. The staff was absolutely wonderful to my father. Everyone was great and very accommodating to my dads needs. They truly care. Amy was great with the other staff putting together a customized care plan for my Dad who was recovering from a brain bleed/cancer and meeting regularly. Can’t say enough about this place each department was great dietary, transportation, PT and OT, nurses, front desk, doctors, and The staff that took care of my father . The facility is very clean and nicely updated.
My father went to the Fulton Center for Rehab. This place was fantastic, communication is the key and if you need help or answers someone was always willing. Everybody was wonderful the doctor, social worker, nurses, aides , therapy, and transportation . Thank you all so much for all your help I have nothing but admiration and respect for all of you who helped my father. Your all amazing people.
This entity took over for the county, as the county wanted to save money by selling. Then the new company wanted the assessment lowered to a laughable amount right after purchase, to not have to pay their usual tax bill. I have lived near this facility all my life and have had loved ones placed there during both ownerships. I can say I have seen a decline in patient care due to staffing and cutting costs since this company took over, and I have heard past & present staff complaining about it. And I wish the 2014 outside photos used in this article were like the scene this past year, but the fence rotted and fell down and the apple orchard and other eastern side (where a cemetery is) were left in 4 foot weeds this year. If you have to put a loved one there, make sure you visit every day, and rotate times of visitation.
Before you place your loved one anywhere, go in and sit for several hours, especially at night. This place is VERY under staffed! Dirty, stinky, from not changing patients. Uninformed about patients individually needs. Patients are left unsupervised for extended periods of time. I dont want to go into detail of my mothers experience, other than just horrible!
My mother was placed in the The rehabilitation part of this facility. They placed her in the dementia unit, and left her in a bed for over four days straight without any therapy at all. On top of that they did not Give her water pills for multiple days causing her to retain water in excess of 20 pounds complicating her congestive heart failure. they did not have the proper medicine before her so instead of getting it they just allowed her to go without it.
Facility should be shut down. Poor medical care and care in general. Food is absolutely terrible. Clothes always going missing in laundry. Personal items stolen. Take forever to get response when call for help. Many times no one to be found . The only department that deserves praise is activities and the people who work in there. If you have to place a loved one in a facility this is NOT the one. Save yourself and your family from real heartbreak. DO NOT have your loved placed here.PLEASE.
Place is a nightmare! My loved one had to call me at home as no one answered his calls for help and he buzzed light 3 times. They never gave meds on time and they gave the worst treatment possible. Doctor prescribed drugs at 10am and he never got until midnight after we asked 5 times. They left him in his own urine. Then threw him a towel to cleanup. He had bed sores so they handed him a cup of cream to administer himself. They even screwed up the meds! Never send a love one here. If you know anyone that’s there please check on them. Report to the State ASAP!!!
This is NOT the place to send someone you love for rehabilitation. The doctor that came to see my loved one came in for an initial consult at 1:00 AM after the patient had been there for days. You would think that he would have the sense that patients in a rehabilitation facility would be sleeping at that hour- These patients who are in pain, have had their night time medications are exhausted from therapy and may be a little less than alert during the night! When I asked why he came into see the patient at that hour, the “Team” of providers said that the doctor had been spoken-to about that before, and it would not happen again. So he showed up between 9:00 and 10:00 PM on two other occassions, when the patient was already having difficulty sleeping at night. AND he refused to prescribe Necessary medications that were previously prescribed by the patient’s Specialist. On discharge, 13 medications were prescribed, and 5 of them were incorrect; he, himself ordered those medications, and then they failed to send to the pharmacy. The patient has difficulty walking, so I went in to get the scripts and waited a half hour only to be told they did not receive the e-scripts. I had to call Fulton Center 3 times (while at the pharmacy) to get someone to answer the phone. My call was transferred and then went to an answering device and I Finally spoke with the Nursing Supervisor who said she would call the doctor to have them sent. I had to drive the patient home after all that, in pain with no medications. I had to make another trip in to the pharmacy that evening. The next day, I found that one of the meds was ordered at DOUBLE the dose of what should have been taken, another med was half of the originally prescribed dose, and two meds that were taken daily while in-patient were not ordered at all even though they were on the Discharge Instructions as taken daily. The issue is still not cleared up 24 hours later, and three more calls to Fulton Center. One of the things needed was needles for insulin for the diabetic patient. Now its even later, and the patient still has not gotten the needles OR the non-narcotic pain med that is needed. There were MANY other issues, but this should suffice. If your loved one goes here, ASK QUESTIONS and WATCH LIKE A HAWK!!! Don’t take their word for it that things are being done, because they may not be.....
If I could put zero I would. my sister was there. They paid no attention to her at all. Her TV was broke the whole time she was there. She cried a li . I called them. It did no good she asked for something they didnt bring it. She couldnt wait to go home.the food was awful and most of the time she couldnt eat it

Thank you to all the staff for your kindness, this is very much appreciated. All the facilities are available. The staff is pleasant, and the place is well maintained. The nurses are very careful, and the management takes care of the needs of the residents. Thanks for the facility..

2021-12-31My husband went there for rehab after cardiac surgery. He spent one night and I brought him home. We were told to go buy bottled water because their water was dirty and you couldnt drink it. The staff was extremely nice but obviously not nearly enough for the amount of patients. The rooms are very crowded and dirty. The place literally stinks. If you really dont care about a person this is the place to put them. It is truly a substandard place.

2022-01-17My Dad treated the nurses and aides like gold! Often times bought them pizzas. What did he get in return....TREATED LIKE S...T! At one point, BROKE HIS BACK from not listening to him (never walked again after that). Neglect and abuse at its finest! When he died....oh lol...lied to me that morning too. Told me he died of pneumonia when it was a silent heart attack (to this day I still question whether it was really silent). Dad only knows.Ive turned down opportunities to work there in my late fathers honor! Would not recommend this place to patients OR aides/nurses looking for work.

2022-05-14They feed my friend (and presumably other residents) “food” that I wouldn’t give to a feral dog. Good staff are far between, most are apathetic at best…and I won’t even say what happens with the worst. If you want to dump someone you can’t stand while you wait for them to die, by all means. If you like them more than gum stuck to the bottom of your shoe I’d suggest looking elsewhere.

2022-04-06My brother is there and I live in Nevada. I have been trying to get ahold of someone…. anyone because I have some serious concerns about his health and care. No one answers the phone at the nurses station. I called the main switchboard yesterday and asked to speak to someone in charge…. And was hung up on. I left a message for his social worker 9 days ago with no return call. Honestly?

2022-05-03Bring your family here if you wanna watch them dieHeres a list of what I saw/heard while my family member was there-dirty room (floors were dirty didnt look cleaned in weeks. Portable toilet not cleaned out-patient not getting care they needed (aids never came for ring of button)-staff seemed like it was out of there way to take care of a patient. Begged family if they would stay longer because then they didnt have to sit with the patient while we were there-patient was suppose to be getting physical therapy but according to patient they never did so. Most they would do was sit her up in bed (got a call onw day they sat her up and left her 3 hours later when we arrived she was still in the same spot)-staff always smelled like cigarettes-family member wanted to tell us things but was to afraid of anyone hearing- plus alot more dont have tike to write it out. Multiple calls have been put into the DOH moral of the story dont bring your family here If you want them to get better

2022-01-05God bless these amazing doctors and staff for their amazing job in getting our mother back on her feet after a three month stint in three different hospitals. She had suffered multiple strokes and had a long battle with Covid. Once she arrived in this wonderful rehabilitation center, things started getting better immediately. We cant thank you fine folks enough for all of your support and guidance. Thank you,Thank you, thank you...and thank God there are amazing people like you to care for the weakened and vulnerable folks that are suffering.

2021-10-01This facility is suffering in staff like all medical facilities and nursing homes and despite that has maintained good communication with me and cared for my loved one. Ive been having a really horrible experience with care for my mother as facilities are predatory and you must advocate for your loved ones care and its been so rough! This facility has been a godsend.Clean, caring staff and available doctors NPs and nurses to ensure your loved one is watched over.

2021-09-10Would not trust them to take care of a fake plant. Do not trust them with your loved ones. Yakov is a terrible person and should be held responsible for his gross negligence. Darla at the front desk is the worst kind of liar. The social workers and patient advocate do not return calls or have the basic ability to dial a phone. The doctor is medically incompetent and should have his license revoked. The head nurse is a liar, incompetent and does not have any shred of basic human compassion. The pt and ot department should be stripped of any and all licences. Again this is the worst possible place for your loved ones. The Fulton county jail would be one million times better than this hell on earth. DO NOT SEND YOUR LOVED ONE HERE. Not to mention they have unreported COVID outbreaks weekly.Update. They are now asking me to email me them. I have called corporate and have spoken to Yakov. I stand by my review. Poor excuse for humans. Your loved ones would receive better care at Rykers island correctional facility.
