Staff claims they are short handed, so short handed that my best friend has been sleeping in a chair for 2 weeks because they dont have time to put her into bed, I just got off the phone with her,she needs to go to the bathroom, no one has come though she has called at least an hour,she needs a drink, they refuse to give her a drink because they dont want her to have to urinate, 3 weeks ago she fell out of bed,had to go to the hospital, add insult to injury I spoke to my Dad this morning, my Aunt was there also,come to find out she fell out of bed 3xs. This is the way you treat sick,older unable Love ones!!! Shame on you!!!
This is by far the best facility that I have ever been associated with. I have been witness to a great deal of residents who go from medically unstable and non-functional to healthy and independent! The staff here is nothing short of miracle workers. They have been especially incredible over the last year plus dealing with the Pandemic and still caring for their residents at the highest levels. If you or someone you know needs a great rehabilitation center, look no further than Momentum at South Bay!!
My 86 year old Mom was admitted to Momentum Rehab after having a 10 day hospitalization for anemia/blood loss. She needed to get her leg strength back since she spent her hospital days mostly in bed and sometimes in a chair. Through Medicare you get 20 days of paid rehab care. I never thought she was going to be there that long so I wasn’t really concerned because with her blood count up she had more energy and strength to progress to walking with a walker as she did before she was hospitalized.If you are looking for your loved one to come home in a timely fashion this is not the place! Unfortunately “The Process” of getting out of this rehab made her stay 23 days! After a week she was walking at her pre-hospital level and I understand that they wanted to get her stronger but unfortunately at 86 there is only so far you can go. If you are recovering from an orthopedic surgery I understand that you may need the extra time but this was not the case!The discharge process should have at least started by week two because they do have regular staff meetings that they discuss discharge and progress of the patients in the program. I felt like my Mom was a prisoner and couldn’t escape! Since she came from an independent senior residence she had to have the appropriate paperwork to be done in order for her to return. Nobody was advocating for my Mom to speed up this process and instead pushed blame and kept saying it is a process to get the paperwork together. This “process” didn’t even get started till she was there 20 days! Did I mention this is a for-profit institution?Since I am out of state, I talked to her social worker a few times and left messages that weren’t returned because they just didn’t want to deal with me anymore instead of facilitating this discharge. My Mom was very upset and crying she just wanted to get home and I was told they would provide emotional support. Really?After talking to Administration and getting an Ombudsman involved she is finally discharged. The Ombudsman was very helpful but the Administrator was not. He said I should be happy that she did so well and was provided such good care. Uhhh……no. Unfortunately there is not enough staff to help. She said she did have some nice nurses and physical therapists but that is not the point. She wanted to go home. She didn’t have a working toilet for 24 hours and was only showered weekly.My job as a daughter (a RN) is to keep my Mom as happy as she can possibly be in the last years of her life. I wish Momentum would have made an effort to do this as well.Dianne Scheich
I’ve been a patient at Momentum over 5 years now. My wife Josephine has been a patient here as well , longer than me. Wonderful staff particularly nursing staff. Food from kitchen is good, but could use some improvements, particularly the temperate of the meals. Nurses and doctors are very good, I have lots of freedom and opportunities to visit my wife, visit the bakery for snacks and relax outside when there is nice weather in the spring and summer time. Productive and energetic staff here, positivity in the building is big thumbs up in my opinion.
Everybody has been terrific. The majority of staff at Momentum have been extremely helpful, and have been there to help me with everything during my stay. Friends of mine that have been patients here in the past also had nothing but nice things to say about the staff and the services provided at Momentum across all department. PT and OT are great, they are always on time, and they push me to get better everyday. If I ever need anything from the kitchen, kitchen staff are quick to get me whatever I request. Recreation visits me everyday to see if I need anything for my room, and housekeeping keep the room very clean and comfortable.
Nursing staff is great. Been a patient here at least 5 years, incredibly compassionate staff workers. Therapists and nurses are very respectful and are professional, courteous kind. There is a variety of meals that are served here, but sometimes the food is not always warm. There are tons of events and programs each week that are fun and engaging, including Bingo, card games, parties, and live entertainment. Staff allow me to sit outside in the sun during the summertime.
I have been blessed with the experience of working at Momentum for over 3 years. There truly are no words to describe the incredible dedication and care each and every staff member gives every single day! To work at a nursing home isnt just a job, it is a calling...and it manifests itself through the hard work and compassion of the staff. The atmosphere at Momentum is always incredibly positive. For our patients, Momentum at South Bay is more than just a rehab center/nursing home...it is a family. Each and every patient at Momentum is treated like family from the very second they arrive. In short Momentum is a shining beacon of hope and excellence, that contains no shortage of heroic and noble staff members ready to give it their all for the patients. During this difficult year of Covid-19, every worker has stepped up to the plate and has given 110%. Even during this dark year, Momentum has shown its true worth and brilliance. It is an inspiring site to witness, and I am so proud to be a part of the Momentum family. Years ago before I was an employee here, my grandfather was a patient at Momentum, and there was no shortage of excellent care, comfort and compassion for him. Every department goes to such great lengths to make all the patients comfortable and happy, and the sacrifices made are beyond the ability to count. BEST JOB I EVER HAD!!!
Staff and nurses are great. Housekeeping does a decent job of keeping rooms clean. PT and OT staff do a wonderful job. Food is a little too salty from time to time, and breakfast is rarely warm. Eggs are cold. I wish they had a higher chair in the room that they have in hospital rooms, a chair that can better support the neck.
So very sorry to have to say, but the rehab was extremely disappointing. Very disappointed with the people In charge. I would never state names, but I pray they never had to experience what my family went through in there facility. I have no idea about the patients that are there long term, but my advice would be to never put your family in this rehab. This was my family’s experience, we can only speak of our experience. A word I could use was neglectful medically. Not equipped to handle anything medical. All I can say is shame on them. Maybe from this they can be honest with themselves and try and improve the facility. 🙏🏻
My experience has been very good. The Bakery is exceptional, couldnt live without it.The Training is very professional and results were obtained. The food is good but a vegetarian menu would be great. Less starch and sugar. Mealtimes are too close together. The Nursing and Aide staff was A+++. They work so very hard. . Concierge/ member services was also very good.The only negative is that I have noticed that most areas are understaffed.For the most part this was a good rehabilitation facility
I have worked for Momentum for almost 3 years and love my job and the people I work with. The team is caring and our residents are our priority. We have a team that works extremely well together and always has the needs of the resident and their familys first. Great place to work and a great place for rehabilitation!!!!
Been working here for 10 years now, I have seen this place grow into something truly special. The rehab in my opinion is the best bar none. They are truly professionals and the staff is amazing. During difficult times this facility knows how to keep moral up and keeps the residents feeling safe and secure. We step up when others fail. I’m proud to be part of this team.
My father went here for rehab in 2020. The facility was reasonably clean. About half the frontline staff were good; the other half weren’t very caring or friendly, didn’t seem to like working there. Some wings were better than others and he got moved around several times. The administrative staff was unorganized, no follow-through, ineffective procedures. All calls go through the switchboard (no direct numbers or extensions) so it’s challenging to reach who you want. Phones ring off the hook. Managers tell you they’ll do something and then don’t. His clothes were lost. He’d show up for visits wearing some else’s clothes and eyeglasses. They lost his orthopedic shoes and refused to replace them without an original receipt (who keeps that?) He was miserable there and we finally moved him to another rehab facility, where he got better care from friendlier staff and actually had some fun. Not impressed with Momentum.
I was brought to Momentum for rehab after breaking my femur May 2017. The PT and OT were wonderful. My aide Rosie was amazing. The daytime nursing staff was also very good and caring.Unfortunately, that is the extent of my praise. Let’s start with the food! It left a lot to be desired and sometimes simply awful. I was young (in my early 50’s) educated and coherent, so I could request what I needed. I was non weight bearing the first few days in the rehab and they needed to use a lift to place me in the wheelchair from bed and then back into bed, Rosie was wonderful doing this but if it was any of the other aides it was horrible and painful. Two aides must be in attendance when using the lift and many times they were too busy conversing instead of paying attention to what they were doing.One of the midnight nurses was non existent. I would need to have my dressing changed on my stitches and some nights it was never done. This caused an infection in my leg and caused me to return to the hospital to have another operation to clear the infection out and a month of IV antibiotics to clear it up.I feel sorry for the elderly patients that can’t fend for themselves and don’t have family near!
I would give this 0 stars if I could! This place is horrible to their elderly patients!! I have no medical training and could probably do a better job taking care of patients than they do. Like when a patient is lethargic with a temperature below 90 degrees, its probably time to do something about it. My family had to call and tell them that something was wrong without being able to see her. Unbelievable. Any person I spoke to had a major attitude problem and it seemed like they only cared about taking our money, than getting our grandmother healthy. Would they care for their own loved ones the way they care for someone elses? HORRIBLE. DO NOT SEND LOVED ONES HERE.
Stay FAR FAR away from this rehab facility if you really care about your loved ones. My grandmother has been here trying to recover from a hip surgery after being transferred from another horrible rehab. This one is NO better. Thanks to covid restrictions you cant even go inside to visit so you have NO idea what is going on. Multiple calls have been made to them. They ignore your requests and refused to release my grandmother to come home !!! By law you CANNOT do this!!!!! Thanks to them my grandmother again went into septic shock and these nurses who need to go back to nursing school kept telling our family shes fine. How the hell do you know that shes fine when she is a dementia patient who cant tell you when things are wrong. If she all of the sudden stops speaking or slurring words. Clearly there is an issue. Also, the hospital noticed she wasn’t producing urine. If you’re changing her diaper, how do these nurses not notice unless they’re not doing their job! She is now fighting for her life at the hospital with not much assurance she will make it. This place will kill your loved ones. Stay the hell away !!!!!!! You should be shut down for neglect !!!!!!!! You also knew something was wrong on December 17th but didn’t do anything or take her to the hospital until TWO DAYS AGO!!!!! Thanks to you my family is completely broken that she may never be coming home ! SHAME ON YOU!!!
For anyone looking into rehabs/nursing homes... please don’t send your loved ones to Momentum. My grandma who has dementia broke her hip in August and has been in rehab since. On December 17th we spoke to my grandma who was screaming in pain, we knew something was wrong and called her nurse. We were given the run around and not only did they refuse to release her to us they also denied her medical care. We were initially told my grandma would be released on the 23rd but then because the social worker Angela realized my grandpa paid until the 30th she wouldn’t release her to us!! We begged to take her home and she threatened to withhold medication, a wheelchair and walker and transport! She convinced us to leave her in their care until the 30th. Finally on December 28th they got her to the hospital where she was in septic shock thanks to an undiagnosed uti and her kidneys were failing. My beautiful, loving grandma is now dying in hospice the moral of my story is do not send your loved ones to Momentum!!
Im still fairly new to Momentum and so far the staff (including management) have been so welcoming and supportive. I have learned so much from my colleagues in these first few weeks. Momentum is a great place to work! They treat the residents like family and provide excellent individualized care. So proud to be part of this team.
I waited a few months to write this review to let the anger settle first. We have used Momentum multiple times over the past 8 years. For my mom (pre Covid) it was great. She rated it 5 stars. For my dad this spring, it was miserable. My dad came recovering from a UTI that left him mentally altered. He couldn’t advocate for himself and due to Covid restrictions we couldn’t see him. He began not being able to keep food down (even though the nurses would tell me he was fine when I called for updates). No matter how many times I begged the staff that this wasn’t normal for him, it fell on deaf ears. He lost almost 40lbs before we figured out his cortisol levels were dangerously low. If your family member can’t advocate for themselves this might not be the place for you. Communication is horrendous, especially from the nursing staff. The bright stars here are Faye in physical therapy and the aides. They were honestly sad to see my dad leave. He was so fortunate to have them. Consider this carefully before choosing them.
We were looking for a facility to help nurse a beloved family member back to health on Long Island, and were underwhelmed with most that we visited.Walking into Momentum in East Islip was a completely different experience.The facility was immaculate, and actually smelled like walking into a bakery. The level of care provided was remarkable, and delivered by people that actually cared.We have recommended Momentum to many others since our positive experience, and have heard nothing but similar experiences.
Momemtum is wonderful in every way, the staff, the aides, the therapists are all great. They care about their patients. The place is clean & the food is good. There is even a small cafe with fresh coffee & cakes baked right there that are available to everyone for free. I was there for 2 1/2 months & I would definitely recommend it for care of a loved one.
I just need to say thank you, my mom was there for rehab after hip surgery and the service was wonderful, therapist are wonderful my mom came home feeling happy with the way they treat her.All RN are very good and help my mom every time she need it ,the nurse assistants are very sweet specially Maritsa,Yolanda,Alberta and others I can’t remember their names.I highly recommend this facility.Thank you and keep the great job 💐
Mom was inadvertently at Momentum for longer than her approved days, I her daughter live in FLORIDA and was on the contact list for emergencies. her insurance was all messed up, there is another sister and brother who live on Long Island and apparently werent contacted or whatever. but my mother has since died with no insurance so there is no one left to get money from but they are now sending the bills directly to me, and now my turn Im handing the next one to my lawyer. PS: the staff taking care of my Mom were very nice and helpful and did take very good care of her, I called every few days and I did talk to her with their assistance.
My grandma passed from COVID-19 here. Just thought I’d put it out there. Considering the DOCTOR wrote something entirely different on her death certificate, and ultimately the death rate from the virus is underreported.During the COVID outbreak, this place did everything they could to make sure my grandmother was comfortable. They let us FaceTime her.Three stars because when she first got here everything was great. She played bingo, they painted her nails, etc. Few times she had bruises and wasn’t showered. If my mom didn’t check up on her she probably wouldn’t have been treated well.
If I could rate a 0 stars I would. It has taken me a few years to get over the traumatic experience my family endeavoured while my grandmother was in their care. I do not know if I can even bring myself to get into all the details still. Please be careful with your loved ones here. More specifically those who are sick especially if they are elderly. The staff is uneducated and does not follow through with patient care. They were warned by other medical attendees about a broken bed that was never fixed or switched which resulted in my grandmother swallowing a dangerous amount of blood from a bloody nose. This lead to one tramautic night in the er followed by days in ICU. After she made it through she then became septic from a bed sore on her back that was not being treated. The staff was uneducated about the signs and symptoms of sepsis. The nurse on staff proceeded to not only yell at me but accuse me of upsetting my grandmother because I did not know what I was talking about. Only after I insisted on a blood test do they call for an ambulance. This is after I, an educator and previous volunteer fire and rescue member, asked several times for them to call one as I knew she had an infection. By the time they called it was too late and she was too far gone. PLEASE look elsewhere if your loved one is elderly or in need of considerable care. These were just the 2 major events that took place while she was in their care. If I continued to reflect on the others; I would reopen the wounds I have spent the last few years trying to forget and forgive.
I have been a Patient at Momentum for about a month now..All of the staff especially the recreation department and Physical Therapy department are incredibly sweet and always around to answer any questions or concerns I might have..everyone is so caring and definitely a welcoming environment..The rooms are spacious and clean..and I Find all my needs are met without hesitation..
They killed my Grandpa. I went to visit and found him dead sitting upright in his wheelchair next to another patient also left in the common room. His oxygen tank was completely empty. I got the police down there and the staff was scrambling and changing stories. Tried to say his oxygen could have ran out after. They also had him on Xanax because he was upset when he was moved there. They didnt just give it to him the once though. That whole week they had him on it! Looks like a really nice place as many do but dont be fooled. If you have family in any rehab be their advocate check on them constantly. Pop in without warning. Constantly ask the staff about their treatment. And if possible, keep them home. My Grandpa didnt deserve to die gasping for air. No one does.
By far one of the BEST skilled nursing facilities in the area. All departments from rehab staff, nursing, housekeeping, to dietary the entire facility is filled with caring and passionate employees. I’ve had multiple friends/neighbors/family members here and would highly recommend!
I wish I could give it 10 stars. My grandfather-in law was there after being hospitalized for a heart condition. The rehab and nursing staff went above and beyond for him. Everyone was helpful and knowledgeable. He was 10x better than he was before entering here.Two thumbs up.
I have been here less than 24 hours . So far there has been not one pleasant experience. I am Diabetic and the kitchen sends me juice with 22 grams of sugar. I havent eaten since I got here because the kitchen keeps sending me the wrong diet. The aids all seem to be mentally handicapped. As soon as I am finished with this review I am going to research rehabs in my area to find one with some intelligence

2022-05-20I have had experience with several rehab facilities in Suffolk county, so when I had to choose one for my mom, I was skeptical. But the experience we had at Momentum was top notch. From admission to discharge, the experience was amazing. Nursing care was as good as it can be considering the nursing shortage country-wide. Rehab was incredible. Hi-tech gym and experienced therapists that communicated with you regularly, food was fine, on site bakery was a treat. Recreation activities were offered every day and my mom had so much fun and made several friendships with other rehab patients. Music was very entertaining and back yard patio is beautiful. I would recommend Momentum to all my family and friends.

2022-04-08My grandmother stayed at Momentum for ~1 month. During that time her stay was nothing less than fantastic. The staff are kind, patient, and caring. My grandmother was a difficult patient but they always practiced patience and kindness. In particular the staff in the 2E wing really helped us make the best out of a tough situation.They were on top of hygiene (the facility is extremely cleanly), the food was always good and in large portions, and the televisions hundreds of channels.I highly recommend Momentum.

2021-12-30My mother has been in and out of different nursing facilities in Suffolk County for the past 3 years. I have to say Momentum has been the best one. My mom received wonderful care, excellent rehab team,, they listen to your concerns and they care. I have not had many good experiences with several other facilities. Especially during the pandemic, Momentum does their best to accommodate families and my mom was very happy there. I would recommend this facility to my family and friends.

2022-02-15Both myself and husband Carmelo have been patients at momentum at different times. I can honestly say we were both treated with the utmost care and dignity. And respect and understanding. I can say that the nursing, aids, therapists, DR and nursing care at desk treated us with the utmost care. The therapy dept was amazing. Very organized and professional. I knew he was safe and cared for when I left to go home. I never had any doubt or fear leaving him. Myself included. I was in a great facility. I would recommend momentum to everyone. No matter what you are there for.

2022-06-27My name is Denise, I am very proud knowing Momentum of South Bay rehab center helped me recover. I truly enjoyed and respect the entire family of Momentum. I appreciate the patience and frustration and struggle that Momentum helped me with, to get me back on my feet. The entire staff is wonderful and I consider them my family. I would recommend Momentum to everyone that needs assistance, this is my second time around. I was broken and they helped me get better spiritually, emotionally and physically. 2022-03-10This place is the worst place to work. The union is zero help. Managers are so uneducated. Looks like the staff posts reviews on their own page from what it looks like to try to bump up their score. Pay closer attention to the 1- and 2-star reviews over the 5-star reviews that seem too good to be true... Also, payroll is atrocious. I did not get paid correctly for every week that I worked there.

2022-04-29Managed to lose both dentures of my bedridden mothers on week one and week two of her being there and are telling me they are not responsible and we have to pay for new ones. Other than that all staff is nice and cared for my mom well.

2020-09-26Having been a patient here at Momentum Rehab for almost 3 months now. I think I have been able to get a keen perspective into the facility. I shattered my leg and had to undergo a few surgeries, recuperate, then go for another surgery. The Wound Care team has tended to my wounds, even my grafts, with the greatest concern. He has worked patiently with me as I healed many times in excruciating pain. The Physical Therapy and Occupational Therapy I have received here has been beyond what I expected. I have had the same two people assigned to me, which I find awesome. There was a perfect balance of patience and pressure to push myself. With every setback or progress, they adjusted my workout to move me forward. In all honesty, I wish I could take them home with me, as I still have a long road ahead, but I can’t so instead they worked very diligently with me to help achieve the goals I need in order to go home with confidence.I love the fact that I have the same nurses and aides. It has allowed me the care that I need without having to explain my unique situation over and over again. They began to feel like my guardian angels. They felt empathy when I cried in pain, when I cried in frustration, and never hesitated to contact the doctor when I felt something wasn’t right. They certainly were in here working out of love, love for people, and you could feel it.It seems that even the people who might be overlooked, like the cleaning staff ( who come in with smiles not only ready to clean my room but go the extra mile to make sure I’m happy), the kitchen staff ( who never complained when I wanted to adjust any of my meals, which was usually everyday ) or those who work in concierge ( who must have super energy because they make a million trips to peoples rooms making deliveries or checking in on us) put their heart and soul into making sure the patients are happy and well taken care of. You definitely develop a wonderful relationship with those who work with you every day. My injury has caused me to feel depressed. They even have an awesome psychologist that has helped me through my negative thoughts. She seems to pop in just when I need her and doesn’t get insulted if I don’t. I would imagine she is one of the gifts this facility has that many may not be aware of.Now, being here as long as I have, I think I have also seen some faults. The nursing staff along with the aides seem to have suddenly become overworked. In my opinion, I think with an influx of patients they have become understaffed. This has led to Great Nurses and Great Aides not being able to give equal attention to their patients. Now with visitors, arranging that privilege along with the Covid restrictions, has pulled people from their normal jobs, like in the concierge department, to now having to oversee the visitors outside. I feel if more staff can be hired and trained for these new situations, it will be the facility I fell in love with.I will say though, that anytime I have complained, because of course when you’re in a facility for three months you’re going to find something to complain about, in Momentums defense, they have always tried and usually succeeded in making me, the patient, happy.🦋 I am editing this, as I am now leaving in 4 days. It’s been 3 months 10 days. I thought I covered everything, I didn’t.The Social Workers have set up everything I need to go home, and I need a lot. They have dealt patiently with me as my anxiety of going back into the real world with a full leg cast, non weight bearing, plus my other health issues that I had, ( I am on SSDI ) prior to coming here. They listened to my concerns and have worked with my insurance, filling out the medically necessity forms, so that my transition home and my remaining healing will be able to be done safely.I am changing my rating from a 4 to a 5 because Momentum and its staff has gotten me through one of the worst parts of my life. I will forever be grateful.

2021-09-09I want to send a heart felt recommendation for this facility. My mom was mentally not ready for a nursing home. Physically was a different story. The staff, was so supportive and so caring that she was able to adapt very nicely. The facility has so many programs to offer stimulation and community. Mom was a recreational therapist in her prior life and the recreation team loves her input. She feels important. How can I say it better? She feels like she makes a difference in the lives of the staff and community around her by celebrating their milestones, and they hers in turn! Each of the departments, social work, physical therapy, nursing, patient care, recreation, nutrition and the administration staff are 100% engaged in the patient and their family. My grandmother lived to be 100 in this facility with great love and our story continues with my mom today. I read something that said if you work in a nursing home you should remember that you are working in their (patients) home. Never a more true statement than the attitudes of this staff. God Bless you all. So many angels amongst you and how grateful are we to be blessed to have you caring for our Mom!

2022-01-22Its like someone else said in this review if I could give it 0 I would. I wouldnt put my dog into this place. My brother had a bracelet saying Fall risk he had a fractured vertibre and had a brace on his neck.he was always up and about when I went into his room. I even video taped it because it happened so often I understand that they are low staffed . but he was in the hallway one morning walking down holding the railing because he didnt know where he was going or where he was A woman visiting her mother found him that way and took him to the nurses station. John is down syndrome and hes almost non verbal so he couldnt tell me what was going on so I went every night to make sure they were taking care of him and our other brother went during the dayI wanted him out of there but just like someone on this page wrote they would not ok it and I was told that they would not let us have the equipment we needed through medicaid because they didnt think he was ready to go home..When he was finally released home..it took 3 weeks to get PT and diapers through medicaid because they did not release john on paper saying he was home to medicaid so when I called medicaid to see what was going on they told me he was still listed in the nursing home. If it wasnt for 2 social workers in medicaid who expedited his case john would still be waiting for PT and continence supplies.When they gave me the release papers on December 31st I saw that the doctor wrote that John did not have vision impairment.. John is BLIND in one eye. He also checked off that he did not have have incontinence.. John has been wetting himself and the bed since his release. The dr. Did not speak with me the whole month john was there and the only reason I got to speak with the PA was at my request. Seems the dr. Is only there once every 2 weeks, but I assume his phone works. I was told that PT did not feel that john needed bed rails..he fell out of bed last week..2 days before I took him home I saw him in the hall without a mask..the home has covid there..As I am writing this I just received another rail for the shower..they had only sent 1 originily . I still cannot believe they got 5 stars
