I have had experience with several rehab facilities in Suffolk county, so when I had to choose one for my mom, I was skeptical. But the experience we had at Momentum was top notch. From admission to discharge, the experience was amazing. Nursing care was as good as it can be considering the nursing shortage country-wide. Rehab was incredible. Hi-tech gym and experienced therapists that communicated with you regularly, food was fine, on site bakery was a treat. Recreation activities were offered every day and my mom had so much fun and made several friendships with other rehab patients. Music was very entertaining and back yard patio is beautiful. I would recommend Momentum to all my family and friends.

My grandmother stayed at Momentum for ~1 month. During that time her stay was nothing less than fantastic. The staff are kind, patient, and caring. My grandmother was a difficult patient but they always practiced patience and kindness. In particular the staff in the 2E wing really helped us make the best out of a tough situation.They were on top of hygiene (the facility is extremely cleanly), the food was always good and in large portions, and the televisions hundreds of channels.I highly recommend Momentum.

My mother has been in and out of different nursing facilities in Suffolk County for the past 3 years. I have to say Momentum has been the best one. My mom received wonderful care, excellent rehab team,, they listen to your concerns and they care. I have not had many good experiences with several other facilities. Especially during the pandemic, Momentum does their best to accommodate families and my mom was very happy there. I would recommend this facility to my family and friends.

Both myself and husband Carmelo have been patients at momentum at different times. I can honestly say we were both treated with the utmost care and dignity. And respect and understanding. I can say that the nursing, aids, therapists, DR and nursing care at desk treated us with the utmost care. The therapy dept was amazing. Very organized and professional. I knew he was safe and cared for when I left to go home. I never had any doubt or fear leaving him. Myself included. I was in a great facility. I would recommend momentum to everyone. No matter what you are there for.

My name is Denise, I am very proud knowing Momentum of South Bay rehab center helped me recover. I truly enjoyed and respect the entire family of Momentum. I appreciate the patience and frustration and struggle that Momentum helped me with, to get me back on my feet. The entire staff is wonderful and I consider them my family. I would recommend Momentum to everyone that needs assistance, this is my second time around. I was broken and they helped me get better spiritually, emotionally and physically. This place is the worst place to work. The union is zero help. Managers are so uneducated. Looks like the staff posts reviews on their own page from what it looks like to try to bump up their score. Pay closer attention to the 1- and 2-star reviews over the 5-star reviews that seem too good to be true... Also, payroll is atrocious. I did not get paid correctly for every week that I worked there.

Managed to lose both dentures of my bedridden mothers on week one and week two of her being there and are telling me they are not responsible and we have to pay for new ones. Other than that all staff is nice and cared for my mom well.

Having been a patient here at Momentum Rehab for almost 3 months now. I think I have been able to get a keen perspective into the facility. I shattered my leg and had to undergo a few surgeries, recuperate, then go for another surgery. The Wound Care team has tended to my wounds, even my grafts, with the greatest concern. He has worked patiently with me as I healed many times in excruciating pain. The Physical Therapy and Occupational Therapy I have received here has been beyond what I expected. I have had the same two people assigned to me, which I find awesome. There was a perfect balance of patience and pressure to push myself. With every setback or progress, they adjusted my workout to move me forward. In all honesty, I wish I could take them home with me, as I still have a long road ahead, but I can’t so instead they worked very diligently with me to help achieve the goals I need in order to go home with confidence.I love the fact that I have the same nurses and aides. It has allowed me the care that I need without having to explain my unique situation over and over again. They began to feel like my guardian angels. They felt empathy when I cried in pain, when I cried in frustration, and never hesitated to contact the doctor when I felt something wasn’t right. They certainly were in here working out of love, love for people, and you could feel it.It seems that even the people who might be overlooked, like the cleaning staff ( who come in with smiles not only ready to clean my room but go the extra mile to make sure I’m happy), the kitchen staff ( who never complained when I wanted to adjust any of my meals, which was usually everyday ) or those who work in concierge ( who must have super energy because they make a million trips to peoples rooms making deliveries or checking in on us) put their heart and soul into making sure the patients are happy and well taken care of. You definitely develop a wonderful relationship with those who work with you every day. My injury has caused me to feel depressed. They even have an awesome psychologist that has helped me through my negative thoughts. She seems to pop in just when I need her and doesn’t get insulted if I don’t. I would imagine she is one of the gifts this facility has that many may not be aware of.Now, being here as long as I have, I think I have also seen some faults. The nursing staff along with the aides seem to have suddenly become overworked. In my opinion, I think with an influx of patients they have become understaffed. This has led to Great Nurses and Great Aides not being able to give equal attention to their patients. Now with visitors, arranging that privilege along with the Covid restrictions, has pulled people from their normal jobs, like in the concierge department, to now having to oversee the visitors outside. I feel if more staff can be hired and trained for these new situations, it will be the facility I fell in love with.I will say though, that anytime I have complained, because of course when you’re in a facility for three months you’re going to find something to complain about, in Momentums defense, they have always tried and usually succeeded in making me, the patient, happy.🦋 I am editing this, as I am now leaving in 4 days. It’s been 3 months 10 days. I thought I covered everything, I didn’t.The Social Workers have set up everything I need to go home, and I need a lot. They have dealt patiently with me as my anxiety of going back into the real world with a full leg cast, non weight bearing, plus my other health issues that I had, ( I am on SSDI ) prior to coming here. They listened to my concerns and have worked with my insurance, filling out the medically necessity forms, so that my transition home and my remaining healing will be able to be done safely.I am changing my rating from a 4 to a 5 because Momentum and its staff has gotten me through one of the worst parts of my life. I will forever be grateful.

I want to send a heart felt recommendation for this facility. My mom was mentally not ready for a nursing home. Physically was a different story. The staff, was so supportive and so caring that she was able to adapt very nicely. The facility has so many programs to offer stimulation and community. Mom was a recreational therapist in her prior life and the recreation team loves her input. She feels important. How can I say it better? She feels like she makes a difference in the lives of the staff and community around her by celebrating their milestones, and they hers in turn! Each of the departments, social work, physical therapy, nursing, patient care, recreation, nutrition and the administration staff are 100% engaged in the patient and their family. My grandmother lived to be 100 in this facility with great love and our story continues with my mom today. I read something that said if you work in a nursing home you should remember that you are working in their (patients) home. Never a more true statement than the attitudes of this staff. God Bless you all. So many angels amongst you and how grateful are we to be blessed to have you caring for our Mom!

Its like someone else said in this review if I could give it 0 I would. I wouldnt put my dog into this place. My brother had a bracelet saying Fall risk he had a fractured vertibre and had a brace on his neck.he was always up and about when I went into his room. I even video taped it because it happened so often I understand that they are low staffed . but he was in the hallway one morning walking down holding the railing because he didnt know where he was going or where he was A woman visiting her mother found him that way and took him to the nurses station. John is down syndrome and hes almost non verbal so he couldnt tell me what was going on so I went every night to make sure they were taking care of him and our other brother went during the dayI wanted him out of there but just like someone on this page wrote they would not ok it and I was told that they would not let us have the equipment we needed through medicaid because they didnt think he was ready to go home..When he was finally released home..it took 3 weeks to get PT and diapers through medicaid because they did not release john on paper saying he was home to medicaid so when I called medicaid to see what was going on they told me he was still listed in the nursing home. If it wasnt for 2 social workers in medicaid who expedited his case john would still be waiting for PT and continence supplies.When they gave me the release papers on December 31st I saw that the doctor wrote that John did not have vision impairment.. John is BLIND in one eye. He also checked off that he did not have have incontinence.. John has been wetting himself and the bed since his release. The dr. Did not speak with me the whole month john was there and the only reason I got to speak with the PA was at my request. Seems the dr. Is only there once every 2 weeks, but I assume his phone works. I was told that PT did not feel that john needed bed rails..he fell out of bed last week..2 days before I took him home I saw him in the hall without a mask..the home has covid there..As I am writing this I just received another rail for the shower..they had only sent 1 originily . I still cannot believe they got 5 stars
