My mother-in-law was admitted on August 13/2021, just 2 weeks ago for respite care. Apon her return home we found that she had not worn any of the clothing that was sent with her. She remained in her pj’s the entire time. I know this because her clothes were exactly the way they were folded by me in her duffel bag. She smelled like cheese which tells me she wasn’t bathed. Her hair was so greasy that it could stand up on its own which tells me it wasn’t washed for days. I received one pair of her pants in a plastic bag which tells me that they were the only thing she soiled out of all she had with her. 3 pair of pants, 9 shirts and 2 pair of pjs were sent . Nothing was touched. To top it off, I had no idea that there were Covid patients there. Seriously???? Not to mention that no one from the family could get any info as to how she was doing and she was put on Morphine. Why? We still don’t know. You all need to step up your game. The elderly deserve better than this!!!
I was extremely happy with the care at this location. Not that long into me being there, they set up a meeting to discuss and update me on all aspect of my care and social work department, rehab department and the RN from my floor were present and kept me informed on my progress and status of going home. I would definitely recommend this place, if you are in need of therapy.Also the food was pretty good 😊
I placed my sister in this facility for short term rehab based on the reviews and photos of the facility. Within 24 hours of her being there, I was told, by two different staff members “I’m not dealing with this, it’s time for me to clock out”, and “I am severely unstaffed and your sister is not a priority” My sister is a quadriplegic and dependent for her every need. The first night she didn’t eat dinner because staff didn’t feed her. She rang the nurse bell, no one came for almost two hours, a few times. They are so understaffed that meds are 2-3 hours overdue. I removed her after 48 hours out of concern for her safety. They also kept her medications, they did not give them to her to take home, and did not return them to their pharmacy (L I Scritps). I will be having DOI look into this facility. I would not recommend placing a loved one here, they have too many patients and not enough staff to provide efficient care to those truly in need. I sincerely doubt the validity of the other reviews. I spent 6 consecutive hours trying to reach the nurses station/administration/supervisor, anyone other then a voicemail for the nurses station.
I was here a while back and now a friend of mine is here. He is very impressed with the rehab, food and staff taking care of him. I’m glad that he is happy with my recommendation. If your in need of therapy I highly recommend this place. They renovated the place and the rooms are very nice size.
The employees here are wonderful! Everyone goes above and beyond to make the residents here comfortable, happy and glad to be here. While the food leaves something to be desired, the aides and nurses are always able to find something that is more palatable for the residents. The therapists in PT and OT are amazing and work hard to help everyone get back in their feet. The concierge here will do anything possible to make the residents happy and comfortable. The recreation department has clever and interesting activities for the residents and they, too, go above and beyond to make everyone happy.
My experience here so far is excellent. I have been been here for over a year and I am now a long term resident. Mostly everyone here is good people. The nursing staff is nice. The food here is good and the rehab department is EXCELLENT!! Recreation is my favorite department. Tashina and Justine are amazing nurses to me. The concierge is excellent and Melissa the nurse always is a good listening ear and she is fun. Tommie, Kim and Boomer are very fun to be with and make this place GREAT!!
Wow this facility and the staff are rockstars! I know about this pace form years ago but this place has come a very long way. The staff are extremely professional and the rehab is out of this world. The food has gotten extremely good. Shoutout to the Administrator who gave me his direct number and opened up a reliable line of communication. Thank you Healthcare Heroes!
The nursing staff here is excellent, they treat me good. Desiree is a excellent aide and she always has my best interest at heart. My social worker Stephanie is GREAT!! Valerie in rehab took the time to listen to me and she is working with me to strengthen my weak areas. Anieta always makes sure my room is cleaned everyday. Recreation is good. I like the fact that there are lots of things to do and that they always come to me to let me know what’s going on for the day. The food isn’t good, that’s an area where they need lots of work on.
I am a former employee. The food needs to be improved. Antonette (LPN), she is a very pleasant nurse. When she comes in the room, she always greets me and is very respectful. She takes car of me very well. Happy early birthday to her! Bailey (CNA), she is a great aide. She took care of my while I was on the third floor. She frequently checked on me, was very pleasant and respectful. She was great with conversation. She is a brand new aide, who always has perfect attendance.
I have been in this facility for almost 10 years and there are two excellent things that I can say about it. First, they have a top-notch rehabilitation department here. The equipment is up to date and the physical and occupational therapists are the finest. They do wonderful work In getting people back on their feet (sometimes literally) and back home again. They also keep those of us who are long-term as fit and physically healthy as we are able to be. Second: The recreation department has Been a staple of my existence here. They keep us both entertained and doing engaging activities to keep our ‘brains’ busy. There are parties, music, crafts, painting, anything you can think of or ask for.On the other end of the spectrum, my two biggest issues are, first, the shortage of nursing and aid staffing. For the most part, the medical staff here is excellent but they are stressed and burnt out from working double shifts and having to care for more residents than they can give the care and attention they deserve. Some of this is reflected in the companys inability to hire and retain enough staff because they dont have to. There are no laws regulating a ratio of nurses and aids to residents. I am working on moving our state and federal representatives toward introducing a bill to address this important subject. The other issue I have is something that, again, is not limited to this facility in particular. Residents , mostly long-term, slowly begin to lose their own individual identities and they become a commodity - a resident rather than the unique person they were when they arrived. Unfortunately, workers forget that we are very much like they are and deserve the same respect and dignity that they would expect for themselves. Simple things like keeping their voices down while congregating outside a residents door when they are sleeping in the early hours of the morning or napping in the afternoon. Showers are scheduled and given twice a week at a time that is convenient to their needs without regard for our former habits - as are our mealtimes, bedtimes, etc. All of those things which they would not tolerate if it were happening to them. Scheduled in-service discussions that address this issue is necessary to remind them that residents have rights to respect and dignity just as they do. These issues are not limited to The Grand. They are a consequence of the growing model of corporate health care.
This is a nice nursing home to live in. I love the cleanliness. It doesn’t feel like I’m in a nursing home. There are a lot of outlets here for us. The food I can do without. The nurses and the aides are alright with me. They never mistreat me here and everyone is pretty nice.
I have been here at river valley for about two months now. The nursing staff here is very helpful and the therapy department is helping me get my strenghth back. The 4th floor nursing staff is AMAZING!! The Recreation staff is fun and always have something planned for me to do to pass my time. The food at times needs a little work but it is a nursing home so its expected.
I’ve been meaning to post this for a while but now is the perfect time after seeing what The Grand did for their residents, their residents families and their employees for Thanksgiving. My grandma has been there for almost 3 years and each year has been a better and better experience. Yesterday The Grand prepared a beautiful luncheon for the residents and the residents families and Michael from Administration handed out homemade delicious apple pies on behalf of the owner of the company. Believe it or not The Grand actually cares about their residents and are always looking for ways to improve the quality of their care and the quality of their overall experience. It’s refreshing to know there are places like The Grand. Way to go Michael, Teresa and the entire team!
They admitted my grandmother here while actively on a COVID lockdown, which they had been in for FIVE DAYS before she arrived. They allowed a vulnerable old woman into this facility knowing that there were active COVID cases here. Somebody needs to be held accountable for this.
LPN Melissa on 5 is an excellent nurse. She cares about her residents. Maggie in social work is a sweetheart and always listens to me. Diane is cool. The CNAs Juanita and Patricia are good to me too. The concierge is thorough, she gets the job done and is compassionate. Boomer treats all the old ladies with respect and love. He be looking out for me too. Dan & R.L in therapy are the truth. My speech therapist Erin is always holding it down.
My father is currently in this place and he’s having an awful time. Requests his meds and doesn’t get them until 2 1/2 hours. How can you possibly stay ahead of the pain if this is the service he’s getting? Recently, he asked if he could get a cup of coffee every morning and he was told they were out of coffee! How does that even happen? Run to the store and get some for God’s sake! He’s supposed to receive therapy everyday and he’s been there over a week and only received two sessions. This is unacceptable. Stay away from this place as their online brochure is a lie! Very disappointed to have to write this. I hope his dr can get him the hell out of there!
If you want to work short staffed and have ZERO help from the supervisor then this place is great. It is disgusting how this place is ran. The staff come in whenever the hell they feel like it. Never on time. ZERO coverage. The residents suffer. If you want your licenses at jeopardy then this is the place. TERRIBLE!
I don’t know how people could be so heartless. My grandmother was there for 3-4 weeks, I sat with her for 3 hours the other day you know how many times she rang her bell for help & not one person came to check on her & help her. A day later after she’s in the hospital in a coma with a black eye & broken nose.& they aren’t sure if she’s going to make it. Why? Because she needed help getting up & nobody would help her. She fell on her face when they finally went to check on my grandmother she was blue & was dead.. they didn’t even call my grandfather to let him know what was happening so after my grandfather hasn’t heard from her all morning he called them & they said she was sent the hospital for SHORTNESS OF BREATHE. NOTHING ABOUT HOW SHE HAD A HEART ATTACK, or that they found her on the floor blue.NOTHING. My grandmother wanted to come home the night before my grandfather told her stay one more night & finish your antibiotics.. & cause of lazy workers we don’t knwo if we will ever have her back again. THIS IS THE ABSOLUTE WORST PLACE.. She didn’t deserve this one bit.
I have been with the company for over 7 years and I love my residents, coworkers and administrators. Every facility has it’s ups and down.... but what job doesn’t? I have gained a lot of knowledge throughout the years and and I happy to be an employee. We will continue to strive for five, regardless of the tribulations that have come our way😃💪🏾
I am the husband of someone who spent quite some time at The Grand and can tell you how far they’ve come. The facility is brand new and the entire culture has changed. To know where they were just a few years ago and where they are now is truly remarkable. Give that team all the credit in the world because they deserve it.
A member of my family was sent here after she broke her leg in two places this past May. Initially, I thought that this was a good thing for her & her immediate family, but my assumption turned out to be the farthest thing from the truth.Shortly after her arrival she met a man and started a romantic relationship with him. No more than a few weeks after they started holding hands when walking the halls, they are not just staying together in the same room but have been allowed to sleep in the same bed.I have never in my life ever heard of any medical facility allowing two members of the opposite sex be in the same room together during the day let alone being allowed to move into the same room. It’s ludacris that they sleep in the same bed.This is a place for physical rehabilitation not match making services. People should be focusing on getting well and back to their lives & families, not dealing with a new relationship & all the feelings/emotions that go along with it.In this case it is unfortunate that the man she met is a con man and has already begun taking over her bank and credit card accounts, has alienated her from her immediate family, including her 1st born daughter and 5 month old grandson (her 1st & only grandchild).
I truly think the grand at river valley is an amazing place. Excellent staff very respectful and professional. Especially their rehab center is really awesome. My mother was certainly happy here and we were worry free Excellent service Thank you all for the good experience
Dont bring your love ones to this place, dog food is better than what they serve, and dont let Scott the administrator fool you he and Mike will act as sweet as can be just dont turn your back as soon as you do they will harm you with false statements , the only reason this dump is still open is they do have great PT and OT .
If I could give zero stars that would be my choice. We all know some nursing homes get a bad rap. Some are trying to do a decent job with very little resources and although not fancy, the patients are safe and well fed. This place is the opposite it is unsafe, horrendous food, understaffed and the the safety and general care of your loved one’s is at the bottom of the list. Do not ignore the real warnings being left by other reviewers. The reviews saying this place is wonderful have to be fake. Please try placing your loved one in Lutheran care or one of the neighboring facilities. This place is one citation away from being closed. It was so bad the staff warned us to take our loved one out.

2022-07-11My aunt also was left for 24 hours in a dirty diaper here. This facility is excessively incompetant. My aunt is being overdrugged with pain meds. They lie about having dieticians on staff to create personalized diet plans. The food is atrocious. The phone lines in patient rooms always break. The medical care is non existant. It has been months that we have tried to arrange for a comfry salve for my aunts back pain. They wanted only to use aspercream, we said ok but they could not even manage the preapproved aspercream. They just spontaneously stopped the meds for my aunts macular degeneration. No blood work is done. The aides are often hostile and passive aggressive. I AM GEARING UP TO REPORT THIS FACILITY TO ADULT PROTECTIVE SERVICES. Dont put your loved one in this place. Their disgusting advertising about how they are like a luxury vacation is pathetic and should be illegal.

2022-01-09My uncle has unfortunately been in this facility for years due to being a double amputee on top of many other health issues that delayed his recovery. My family has had nothing but problems with this facility. He has been left in his own fecal matter for hours, the staff refused to help him transfer from his wheelchair to his bed, resulting in him falling, and his clothing and personal items have been repeatedly stolen. My uncle was unable to travel with family for the holidays, so we sent him food back so he could at least enjoy a nice home cooked meal. The nutritionist on site threw his, and many others food items away even after being informed by the nursing staff that the food was indeed still good and had in fact just got there. When questioned her response was The Grand provides good food Are you kidding me? This place is a disaster on all levels. Avoid at all costs!!

2022-01-04If you care for your parents, your family members do not put them in this home! My mom died because she stayed here, based on the medical examiners report. They did not call me, send me a letter nothing to show any type of sympathy. I blame myself everyday for my moms death. If it wasnt for covid I would have been with her, she would have been able to be home. She was over medicated here in this home. If you love your parent, do not place them here!

2022-01-27Sorry but I wouldn’t send an enemy here. No care, and no contact. The place is under staffed and gloomy. Had to pull my father out after only 24 hours, as he sat in a chair and didn’t move, he was there for PT. When they released him they had him walk down with no wheelchair and no socks on, although he was there because of a fall. This place obviously doesn’t pay their people well.

2021-05-27I’m getting on here to write a review because I have a feeling that this Nursing Home does not get enough credit. Listen up, this place is a great kept secret. They are a extremely STRONG team that truly care about the CARE of their residents. The food is amazing and honestly full of variety, the Social Worker Victoria is super responsive! The Administrator Mark is super hands on with the staff and Residents. The nursing staff work extremely hard to look after the residents and honestly the Rehab is the best Rehab experience I have had yet! Thank you Grand at River Valley!

2021-09-14I requested my mom be sent here from Northern Dutchess when she had to do some rehab. I based my choice on some of the stellar reviews I saw here. It was truly a disaster from start to finish! The only employee who was at all helpful was the girl at the front desk. I was hung up on by someone at the nurses station. My mom was there for 24 hours without food or water. I asked to speak with her social worker who was on vacation and the other part time social worker doesnt work on Wednesday. The director wouldnt call me back. My moms nurse was snappy. I finally had to call in the state to help get her out of there. It was the worst, most stressful situation Ive ever experienced with a nursing home. The kicker was that they didnt inform us of a covid lockdoen and that she would be isolated from her family completely. That should be disclosed so families can make an educated decision about where to send their loved ones. I dont understand how anyone treats this a good facility! Was she just admitted on a bad day? How do you not give food or water until someone calls to ask! Lutherans Care was more than happy to facilitate her move and representatives of the county were on hand to ensure that nothing funny went on like havinger her sign herself out to relinquish responsibility. They have been reported to NYS so Im hoping people think twice before sending loved ones here.

2021-11-30If your loved one has any sort of dementia, this is not the place for them. A constant watch must be kept to make sure you are getting all your meds...and the right ones. The places Ive worked in or the last nursing home I was in, the nursing staff got used to regular medication regimens and would question any discrepancies. Here, not so. If it isnt in the computer its out of their head...whether or not youve been on it long term. If your loved one cant keep track of their own meds, dont expect these people to cover for their inability to do so.I have nothing but the best reviews on the rehabilitation staff!! Both P\/T and O\/T are very knowledgeable and dedicated. All their responsibilities, towards me, were exceeded. If you or your loved one needs rehabilitation, this is an excellent choice, as long as they ONLY need rehabilitation.The nursing staff is dedicated too, but only in as much as is humanly possible. They are too short staffed to perform even the minimum care needed. In fact, they are dangerously understaffed in general. As a RN myself, I think administration could help alleviate this. They can do this by paying the bonuses due to staff for working so undermanned!! They could avoid the call in rates by paying the government funded 30.00 per shift COVID bonuses instead of using this money to buy yet another nursing facility!!Im here strictly for wound care. I came with a large wound that the doctor is utilizing a wound vac to help heal. Its meant to be on continuously and only to be changed three times a week...never off for more than two hours. But they took it off on Saturday and never put it back on until Wednesday! The surgeon noticed the lack of use due to the condition of my wound!! This happened on two separate occasions due to the staffs lack of knowledge on the use and care of a wound vac, yet after I arrived, they now several people with wound vac. I was here 6 weeks before an in service was given to the nurses on how to change a wound vac! Luckily, the unit manager took the time to learn how to do itTheir response to my initial review, was to call them to straighten this out, but phone calls are not returned or simply not accepted. The administrators favorite saying seems to be, it will all work out in the end.He asked me to reevaluate and write a new 5 star review.... I did reevaluate it, and it gets the same 1 star rating, but I have included more information as to why my score is so low.I spent the first 4-5 weeks washing my clothes out in the sink to avoid having to put labels on my clothes. But, its colder now, so heavier clothing needs to be worn. I finally broke down and let them label and wash my clothes....BIG MISTAKE!! I cant even describe the condition my clothing came back in!! I complained to administration. I tried to show him online receipts showing these items are new. Two items had never been worn, but came back stained?? I just received these two items 2 days before the washing...never had a chance to wear them. But they were washed with the rest of the clothing being labeled. Administration claimed that he would follow up. He showed up, but didnt follow up. The damaged items should have been replaced...I only asked that one be replaced, but that didnt happen. They also didnt flinch at my report of items missing from my room. They couldnt be bothered to investigateAs any place like this, the food is not very appetizing. But attempts are made to meet the needs of all

2021-04-02I am happy that I followed my friends advice and tried this place, Administration was extremely helpful throughout my journey here, the staff are smiley, friendly and caring. Social Work always answered my calls and put me in touch with the right person. The gym is beautiful and the rooms are very spacious and renovated. The food was good and loved the “Grand” sandwiches they would make upon request. I hope I never need subacute rehab again but I’m glad I found a place that I feel comfortable and at home with.

2021-04-02My sister recently came to this facility for short term rehab, and we were thrilled with how well she progressed here. We are extremely thankful to the entire team. A special thank you to Administration, Val the Rehab Director and Victoria from Social Work, as well to all the hard working nurses.

2022-01-27Im truly honored to be a part of a team of extraordinary people that do all they can to go above and beyond for the people who need help the most everyone that is a part of the staff is truly amazing and wonderful to work with they truly put residents first in healthcare we need more people like the staff at the Grand rehabilitation center at River valley one of the best rehabilitation centers in the Hudson valley area that is helmed and led by Jonathan Holan truly wonderful person an amazing mentor and someone who truly values the care of the residents that are in this facility speaking of someone whos a part of the team and newly a part of the team I can strongly say that working here has made my life my confidence and who I am as a person better cuz I feel like Im truly adding to the lives of people who need it the most so when it comes to care I believe theres no rehabilitation center besides our sister location
